A cataract is a clouding of the eye’s natural lens, which lies behind the iris and the pupil. The lens of the eye is mostly made of water and protein. The protein is arranged in a precise way that keeps the lens clear and let’s light pass through it.But as we age, some of the protein may clump together and start to cloud a small area of the lens. This is a cataract, and over time, it may grow larger and cloud more of the lens, making it harder to see.

Cataracts are the most common cause of vision loss in people over the age of 40 and are the principal cause of blindness in the world.

Hazy or blurred vision may mean you have a cataract. A cataract may make light from the sun or a lamp seem too bright or glaring. Or you may notice when you drive at night that the oncoming headlights cause more glare than before. Colors may not appear as bright as they once did.

Causes: Besides advancing age, cataract risk factors include:

  • Ultraviolet radiation from sunlight and other sources
  • Diabetes
  • Hypertension
  • Obesity
  • Smoking
  • Prolonged use of corticosteroid medications
  • Stating medicines used to reduce cholesterol
  • Previous eye injury or inflammation
  • Previous eye surgery
  • Hormone replacement therapy
  • Significant alcohol consumption
  • High myopia
  • Family history

Types of cataract
A subcapsular cataract occurs at the back of the lens. People with diabetes or those taking high doses of steroid medications have a greater risk of developing a subcapsular cataract. A nuclear cataract forms deep in the central zone (nucleus) of the lens. Nuclear cataracts usually are associated with aging. A cortical cataract is characterized by white, wedge-like opacities that start in the periphery of the lens and work their way to the center in a spoke-like fashion. This type of cataract occurs in the lens cortex, which is the part of the lens that surrounds the central nucleus.

Cataract surgery
Prior to cataract surgery, our optometrist or ophthalmologist will perform a comprehensive eye exam to check the overall health of your eyes, evaluate whether there are reasons why you should not have surgery and identify any risk factors you might have.The procedure typically is performed on an outpatient basis using only local anesthesia and does not require an overnight hospital stay.

In cataract surgery, the lens inside your eye that has become cloudy from cataract formation is removed and replaced with an artificial lens (called an intraocular lens, or IOL) to restore clear vision.Most modern cataract procedures involve the use of a high-frequency ultrasound probe that breaks up the cloudy lens into small pieces, which are then gently removed from the eye with suction.

Recovery: do’s and do’nts
An uncomplicated cataract surgery typically lasts only about 15 minutes. But expect to be at the surgical center for 90 minutes or longer, because extra time is needed for preparation and for a brief post-operative evaluation and instructions about your cataract surgery recovery before you leave.

You must have someone drive you home after cataract surgery; do not attempt to drive until you have visited your eye doctor the day after surgery and he or she tests your vision and confirms that you are safe to drive.

You will be prescribed medicated eye drops to use several times each day for a few weeks after cataract surgery. You also must wear your protective eye shield while sleeping or napping, for about a week after surgery. To protect your eyes from sunlight and other bright light as your eye recovers, you will be given a special pair of post-operative sunglasses. While your eye heals, you might experience some eye redness and blurred vision during the first few days or even weeks following the procedure.

During at least the first week of your recovery, it is essential that you avoid:

  • Strenuous activity and heavy lifting (nothing over 25 pounds).
  • Bending, exercising and similar activities that might stress your eye while it is healing.
  • Water that might splash into your eye and cause infection. Keep your eye closed while showering or bathing. Also, avoid swimming or hot tubs for at least two weeks.
  • Any activity that would expose your healing eye to dust, grime or other infection-causing contaminants.

In safe hands
As frightening as cataracts might sound, modern cataract surgery usually can restore vision lost to cataracts and often reduce your dependence on eyeglasses as well. Modern cataract surgery is one of the safest and most effective surgical procedures.

LASIK is eye surgery that permanently changes the shape of the cornea (the clear covering on the front of the eye). It is done to improve vision and reduce a person’s need for glasses or contact lenses. For clear vision, the eye’s cornea and lens must bend (refract) light rays properly. This allows images to be focused on the retina. Otherwise, the images will be blurry.

This blurriness is referred to as a “refractive error.” It is caused by a difference between the shape of the cornea (curvature) and the length of the eye.

Description of procedure
LASIK uses an excimer laser (an ultraviolet laser) to remove a small amount of corneal tissue. This gives the cornea a new shape so that light rays are focused clearly on the retina. LASIK causes the cornea to be thinner.

LASIK is an outpatient surgical procedure. It will take 10 to 15 minutes to perform for each eye. The only anesthetic used is eye drops that numb the surface of the eye. The procedure is done when you are awake, but you will get medicine to help you relax. LASIK may be done on one or both eyes during the same session.

To do the procedure, a flap of corneal tissue is created. This flap is then peeled back so that the excimer laser can reshape the corneal tissue underneath. A hinge on the flap prevents it from being completely separated from the cornea. When LASIK was first done, a special automated knife (a microkeratome) was used to cut the flap. Now a more common and safer method is to use a different type of laser (femtosecond) to create the corneal flap. We use the latest bladeless lasik technique for Refractive Errors like myopia, hypermetropia, astigmatism and presbyopia. This Femtolaser technique ensures perfect precision in lasik surgery, ensuring optimal visual outcomes.The state of the art lasik platform provides the treatment options of customized Proscan and Zyoptix HD for optimum aberration free distance visual acuity.

The Supracor lasik technique for presbyopic correction is another treatment option for correction of age related near vision difficulties.

The blade lasik is also available and is a safe and effective technique in suitable patients for all the above treatment options.

Photorefractive Keratomelieusis (PRK), Implantable collamer lenses and Collagen cross linking are other treatment options available for those not suitable for Lasik.

The amount of tissue the laser will remove is calculated ahead of time. Once the reshaping is done, the surgeon replaces and secures the flap. No stitches are needed. The cornea will naturally hold the flap in place.

Why the procedure is performed
LASIK is most often done on people who use glasses or contact lenses because of nearsightedness (myopia). It is sometimes used to correct farsightedness. It may also correct astigmatism.

Before the procedure
A complete eye exam will be done before surgery to make sure your eyes are healthy. Other tests will be done to measure the curvature of the cornea, the size of the pupils in light and dark, the eyes’ refractive error, and the thickness of the cornea (to make sure you will have enough corneal tissue left after surgery).

You will sign a consent form before the procedure. This form confirms that you know the procedure’s risks, benefits, alternative options, and possible complications.

After the procedure
Following the surgery:
You may have burning, itching, or a feeling that something is in the eye. This feeling doesn’t last for more than 6 hours in most cases.
An eye shield or patch will be placed over the eye to protect the flap. It will also help prevent rubbing or pressure on the eye until it has had enough time to heal (usually overnight).
It is very important NOT to rub the eye after LASIK, so that the flap does not dislodge or move. For the first 6 hours, keep the eye closed as much as possible.
The doctor may prescribe mild pain medicine and a sedative.
Vision is often blurry or hazy the day of surgery, but blurriness will improve by the next day.
Call the doctor right away if you have severe pain or any of the symptoms get worse before your scheduled follow-up appointment (24 – 48 hours after surgery).

At the first doctor visit after the surgery, the eye shield will be removed and the doctor will examine your eye and test your vision. You will receive eye drops to help prevent infection and inflammation.Do not drive until your vision has improved enough to safely do so. Other things to avoid include:

  • Swimming
  • Hot tubs and whirlpools
  • Contact sports
  • Use of lotions, creams, and eye makeup for 2 – 4 weeks after surgery

The doctor will give you specific instructions.

The retina is the light-sensitive layer of tissue at the back of the inner eye. It acts like the film in a camera. Images come through the eye’s lens and are focused on the retina. The retina then converts these images to electric signals and sends them via the optic nerve to the brain.The retina is normally red due to its rich blood supply. An ophthalmoscope allows a health care provider to see through your pupil and lens to the retina. If the provider sees any changes in the color or appearance of the retina, it may indicate a disease.

Anyone who experiences changes in sharpness or color perception, flashes of lights, floaters, or distortion in vision should get a retinal examination.

Glaucoma is a group of eye conditions that can damage the optic nerve. This nerve sends the images you see to your brain. Most often, optic nerve damage is caused by increased pressure in the eye. This is called intraocular pressure. In glaucoma, eye pressure plays a role in damaging the delicate nerve fibers of the optic nerve. When a significant number of nerve fibers are damaged, blind spots develop in the field of vision. Once nerve damage and visual loss occur, it is permanent. Most people don’t notice these blind areas until much of the optic nerve damage has already occurred. If the entire nerve is destroyed, blindness results. Glaucoma is a leading cause of blindness in the world, especially in older people. Early detection and treatment by your ophthalmologist are the keys to preventing optic nerve damage and vision loss from glaucoma.

Glaucoma is the second most common cause of blindness in the U.S. There are four major types of glaucoma:

  • Open-angle glaucoma
  • Angle-closure glaucoma, also called closed-angle glaucoma
  • Congenital glaucoma
  • Secondary glaucoma

The front part of the eye is filled with a clear fluid called aqueous humor. This fluid is made in an area behind the colored part of the eye (iris). It leaves the eye through channels where the iris and cornea meet. This area is called the anterior chamber angle, or the angle. The cornea is the clear covering on the front of the eye that covers the iris, pupil, and angle.

Anything that slows or blocks the flow of this fluid will cause pressure to build up in the eye.

  • In open-angle glaucoma, the increase in pressure is often small and slow.
  • In closed-angle, the increase is often high and sudden.
  • Either type can damage the optic nerve.

Exams and Tests
The only way to diagnose glaucoma is by having a complete eye exam.

Usually you’ll be given eye drops to widen (dilate) your pupil.
When your pupil is dilated, your eye doctor will look at the inside of your eye and the optic nerve.
You may be given a test to check your eye pressure. This is called tonometry.
Eye pressure is different at different times of the day. Eye pressure can even be normal in some people with glaucoma. So your doctor will need to run other tests to confirm glaucoma.

The goal of treatment is to reduce your eye pressure. Treatment depends on the type of glaucoma that you have.

1. Open-angle glaucoma
• If you have open-angle glaucoma, you will probably be given eye drops.
• You may need more than one type. Most people can be treated with eye drops.
• Most of the eye drops used today have fewer side effects than those used in the past.
• You also may be given pills to lower pressure in the eye.
• If drops alone don’t work, you may need other treatment:
• Laser treatment uses a painless laser to open the channels where fluid flows out.
• If drops and laser treatment don’t work, you may need surgery. You will be put asleep with general anesthesia. The doctor will use a small knife to cut open a new channel so fluid can escape. This will help lower your pressure.

2. Acute angle glaucoma
An acute angle-closure attack is a medical emergency. You can become blind in a few days if you aren’t treated.
• You may be given drops, pills, and medicine given through a vein (by IV) to lower your eye pressure.
• Some people also need an emergency operation, called an iridotomy. The doctor uses a laser to open a new channel in the iris. Sometimes this is done with surgery. The new channel relieves the attack and will prevent another attack.
• To help prevent an attack in the other eye, the procedure will often be performed on the other eye. This may be done even if it has never had an attack. CONGENTIAL GLAUCOMA
• Congenital glaucoma is almost always treated with surgery.
• This is done using general anesthesia. This means the child is asleep and feels no pain. SECONDARY GLAUCOMA If you have secondary glaucoma, treating the cause may help your symptoms go away. Other treatments also may be needed. Prevention You can’t prevent open-angle glaucoma. Most people have no symptoms. But you can help prevent vision loss.
• A complete eye exam can help find open-angle glaucoma early, when it is easier to treat.
• All adults should have a complete eye exam by the age of 40.
• If you are at risk for glaucoma, you should have a complete eye exam sooner than age 40.
• You should have regular eye exams as recommended by your doctor.

The cornea is the transparent front part of the eye that covers the iris, pupil, and anterior chamber. The cornea, with the anterior chamber and lens, refracts light, with the cornea accounting for approximately two-thirds of the eye’s total optical power. In humans, the refractive power of the cornea is approximately 43 dioptres. While the cornea contributes most of the eye’s focusing power, its focus is fixed. The curvature of the lens, on the other hand, can be adjusted to “tune” the focus depending upon the object’s distance.

Corneal injury
Corneal injury describes an injury to the cornea. The cornea is the crystal clear (transparent) tissue covering the front of the eye. It works with the lens of the eye to focus images on the retina.

Injuries to the cornea are common. Injuries to the outer surface of the cornea, called corneal abrasions, may be caused by:

• Chemical irritation – from almost any fluid that gets into the eye
• Overuse of contact lenses or lenses that don’t fit correctly
• Reaction or sensitivity to contact lens solutions and cosmetics
• Scratches or scrapes on the surface of the cornea (called an abrasion)
• Something getting into the eye (such as sand or dust)
• Sunlight, sun lamps, snow or water reflections, or arc-welding
• Infections may also damage the cornea.

You are more likely to develop a corneal injury if you:

• Are exposed to sunlight or artificial ultraviolet light for long periods of time
• Have ill-fitting contact lenses or overuse your contact lenses
• Have very dry eyes
• Work in a dusty environment

High-speed particles, such as chips from hammering metal on metal, may become embedded in the surface of the cornea. Rarely, they may pass through the cornea and go deeper into the eye.

• Blurred vision
• Eye pain or stinging and burning in the eye
• Feeling like something is in your eye (may be caused by a scratch or something in your eye)
• Light sensitivity
• Redness of the eye
• Swollen eyelids
• Watery eyes or increased tearing


First aid for eye emergencies:
DO NOT try to remove an object that is stuck in your eye without professional medical help.
If chemicals are splashed in the eye, IMMEDIATELY flush the eye with water for 15 minutes. The person should be quickly taken to the nearest emergency room.

Anyone with severe eye pain needs to be evaluated in an emergency care center or by an ophthalmologist immediately.

Treatment for corneal injuries may involve:

• Removing any foreign material from the eye
• Wearing an eye patch or temporary bandage contact lens
• Using eye drops or ointments prescribed by the doctor
• Not wearing contact lenses until the eye has healed
• Taking pain medicines
• When to Contact a Medical Professional?
• Call your health care provider if the injury has not significantly improved in 2 days with treatment.
• Corneal ulcers and infections
Bacterial keratitis; fungal keratitis; Acanthamoeba keratitis; Herpes simplex keratitis The cornea is the clear (transparent) tissue at the front of the eye. A corneal ulcer is an erosion or open sore in the outer layer of the cornea. It is often caused by infection.

Corneal ulcers are most commonly caused by an infection with bacteria, viruses, fungi, or a parasite.

Acanthamoeba keratitis occurs in contact lens users, especially in people who make their own homemade cleaning solutions.
Fungal keratitis can occur after a corneal injury involving plant material, or in people with a suppressed immune system.
Herpes simplex keratitis is a serious viral infection. It may cause repeated attacks that are triggered by stress, exposure to sunlight, or any condition that impairs the immune system.

Corneal ulcers or infections may also be caused by:

• Eyelids that do not close all the way, such as with Bell’s palsy
• Foreign bodies in the eye
• Scratches (abrasions) on the eye surface
• Severely dry eyes
• Severe allergic eye disease
• Various inflammatory disorders
• Contact lens wear, especially soft contact lenses worn overnight, may cause a corneal ulcer.


Symptoms of infection or ulcers of the cornea include:

• Blurry or hazy vision
• Eye that appears red or bloodshot
• Itching and discharge
• Sensitivity to light (photophobia)
• Very painful and watery eyes
• White patch on the cornea


Treatment for corneal ulcers and infections depends on the cause. Treatment should be started as soon as possible to prevent scarring of the cornea.

If the exact cause is not known, patients may be given antibiotic drops that work against many kinds of bacteria. Once the exact cause is known, drops that treat bacteria, herpes, other viruses, or a fungus are prescribed. Severe ulcers sometimes require a corneal transplant.

Corticosteroid eye drops may be used to reduce swelling and inflammation in certain conditions.

Your health care provider may also recommend that you:

• Avoid eye makeup
• Don’t wear contact lenses at all, or don’t wear them at night
• Take pain medications
• Wear an eye patch to keep out light and help with symptoms
• Wear protective glasses
• Possible Complications

Untreated corneal ulcers and infections may lead to:

• Loss of the eye (rare)
• Severe vision loss
• Scars on the cornea
• When to Contact a Medical Professional

Call your health care provider if:

You have symptoms of corneal ulcers or an infection
You have been diagnosed with this condition and your symptoms become worse after treatment
Keratoconus is an eye disease that affects the structure of the cornea. The cornea is the clear tissue covering the front of the eye.

The shape of the cornea slowly changes from the normal round shape to a cone shape. The eye bulges out. This causes vision problems.

The cause is unknown, but the tendency to develop keratoconus is probably present from birth. Keratoconus is thought to involve a defect in collagen, the tissue that provides strength to the cornea and gives it it’s shape. Some researchers believe that allergy and eye rubbing may play speed up damage. There is a link between keratoconus and Down syndrome.

The earliest symptom is subtle blurring of vision that cannot be corrected with glasses. (Vision can most often be corrected to 20/20 with rigid, gas-permeable contact lenses.) Over time, you may have eye halos, glare, or other night vision problems.

Most people who develop keratoconus have a history of being nearsighted. The nearsightedness tends to become worse over time. As the problem gets worse, astigmatism develops.

Contact lenses are the main treatment for most patients with keratoconus. Wearing sunglasses outdoors after being diagnosed may help slow or prevent the disease from becoming worse. For many years, the only surgical treatment has been corneal transplantation.

The following newer technologies may delay or prevent the need for corneal transplantation:
• High-frequency radio energy (conductive keratoplasty) to change the shape of the cornea so contact lenses work better
• Corneal implants called intracorneal ring segments to change the shape of the cornea so contact lenses work better
• An advanced treatment called corneal cross-linking causes the cornea to become hard and stops the condition from getting worse. The cornea can then be reshaped with laser vision correction.

Possible Complications
There is a risk of rejection after corneal transplantation, but the risk is much lower than with other organ transplants. Patients with even borderline keratoconus should not have LASIK, the most common type of laser vision correction. Corneal topography is done before laser vision correction to rule out people with this condition.

In rare cases of mild keratoconus, other laser vision correction procedures may be safe to use, such as PRK. For patients treated with CK or CXL, PRK may be safe to perform and may help to further improve the vision.

When to Contact a Medical Professional
Young persons whose vision cannot be corrected to 20/20 with glasses should be evaluated by an eye doctor experienced with keratoconus. Parents with keratoconus should consider having their child or children screened for the disease starting at age 10.

Refractive Errors
A refractive error is a very common eye disorder. It occurs when the eye cannot clearly focus the images from the outside world. The shape of the eye does not bend light correctly and this results in the image appearing blurred, sometimes blurring is so severe that it causes visual impairment.

The four most common refractive errors are:
Myopia (nearsightedness):
Difficulty in seeing distant objects clearly; also known as nearsightedness, myopia is usually inherited and often discovered in childhood. Myopia often progresses throughout the teenage years when the body is growing rapidly. There has been evidence to suggest that outdoor activity may reduce the risk of children developing myopia later. People with high degrees of myopia have a higher risk of glaucoma, cataract formation or retinal detachment which may require surgical repair.

Hyperopia (farsightedness):
Difficulty in seeing close objects clearly; also known as farsightedness, hyperopia can also be inherited. Children often have hyperopia, which may lessen in adulthood. In mild hyperopia, distance vision is clear while near vision is blurry. In more advanced hyperopia, vision can be blurred at all distances.

Distorted vision resulting from an irregularly curved cornea, the clear covering of the eyeball. Astigmatism usually occurs when the front surface of the eye, the cornea, has an asymmetric curvature. Normally the cornea is smooth and equally curved in all directions, and light entering the cornea is focused equally on all planes, or in all directions. In astigmatism, the front surface of the cornea is curved more in one direction than in another. This abnormality may result in vision that is much like looking into a distorted, wavy mirror. Usually, astigmatism causes blurred vision at all distances.

Which leads to difficulty in reading or seeing at arm’s length, it is linked to ageing and occurs almost universally. After age 40, the lens of the eye becomes more rigid and does not flex as easily. As a result, the eye loses its focusing ability and it becomes more difficult to read at close range. This normal aging process of the lens can also be combined with myopia, hyperopia or astigmatism.

Overuse of the eyes does not cause or worsen refractive error. Refractive errors cannot be prevented, but they can be diagnosed by an eye examination and treated with corrective glasses, contact lenses or refractive surgery.

At the onset, common complaints that most patients experience are; inability to carry out simple activities of daily living such as difficulty with driving, reading (particularly small print such as bills or medication instructions) and preparing meals. The symptoms may be so gradual as not to be perceived by the patient as relating to their vision and they actually come complaining of headaches or red, sore, watery eyes. Young children may rub their eyes a lot, have recurrent boils on the eyelids or turn their heads when looking at things (e.g. television) and school-aged children may present with behavioral problems.

A refractive error can be diagnosed by an eye care professional during a routine eye examination. Testing usually consists of asking the patient to read a vision chart while testing an assortment of lenses to maximize a patient’s vision. Special imaging or other testing is rarely necessary. Young children should be referred to an ophthalmologist or optometrist able to provide special pediatric care.

Asymptomatic, low-risk patients (no ocular comorbidity or family history) should have eye examinations at least:
• Every ten years (19-40 years old)
• Every five years (41-55 years old)
• Every three years (56-65 years old)
• Every two years (>65 years old)

Patients at risk of visual impairment (e.g. patients with diabetes, those with cataracts, macular degeneration, glaucoma or a significant family history of these) should have eye examinations at least:

• Every year for all age groups
• Patients wearing glasses should have eye check up
• Every 6 months (pediatric)
• Every year(adult)

At the Eye Care Centre, Refractive errors are managed by our experienced and skilled optometrists (specialists in the diagnosis and management of refractive errors) and ophthalmologists (medically qualified physicians or surgeons).

Refractive disorders are commonly treated using corrective lenses, such as eyeglasses or contact lenses. Refractive surgery (such as LASIK) can also be used to correct some refractive disorders. Presbyopia, in the absence of any other refractive error, can also be treated with glasses, bifocal contact lenses and presbyopic lasik.

Spectacles are the simplest, safest and most cost-effective way or managing refractive errors. Lenses may be spheres, cylinders or a mixture of both. Spherical lenses are characterized by a constant curvature over the entire surface and may be convex (converge light rays, known as plus lenses) or concave (diverge light rays, known as minus lenses). Cylindrical lenses have focusing powers in one meridian only, the orientation of which depends on the patient’s problem. The power of a spectacle lens can be measured using an instrument known as a lens meter. Lenses may have one or more refractive components to them, the latter being known as multifocal lenses. The power needed for each component can be assessed and prescribed separately.

Contact lenses
Contact lenses work on the same principle as spectacle lenses but the space between the lens and the anterior surface of the cornea is reduced to the tear film alone. Orthokeratology is an emerging technique used, whereby a rigid contact lens is fitted to distort the corneal shape in a controlled fashion overnight, so reducing symptoms of myopia during the day.

Surgical Correction
We use the latest bladeless lasik technique for Refractive Errors like myopia, hypermetropia, astigmatism and presbyopia. This Femtolaser technique ensures perfect precision in lasik surgery, ensuring optimal visual outcomes.

The state of the art lasik platform provides the treatment options of customized Proscan and Zyoptix HD for optimum aberration free distance visual acuity.

The Supracor lasik technique for presbyopic correction is another treatment option for correction of age related near vision difficulties.

The blade lasik is also available and is a safe and effective technique in suitable patients for all the above treatment options.

Photorefractive Keratomelieusis (PRK), Implantable collamer lenses and Collagen cross linking are other treatment options available for those not suitable for Lasik.

Oculoplastics, or oculoplastic surgery, includes a wide variety of surgical procedures that deal with the orbit (eye socket), eyelids, tear ducts, and the face. It also deals with the reconstruction of the eye and associated structures. We at RAK Eye Care Center offer a bouquet of services within the scope of oculoplastics like eyelid malposition corrections, lid tumor excision and lid reconstruction, skin and membrane grafting, lid coloboma repair, repair of lax eyelids, lacrimal surgeries for tear duct obstructions, contracted socket correction and botox injections for eyelid and facial twitching.
